Every patient’s prostate cancer journey is unique. From diagnosis through treatment and beyond, the decisions made, and their outcomes, tell a story that shapes not just individual lives, but the future of prostate cancer care.

Prostate cancer remains one of the most common malignancies in men, with around 57,900 new cases diagnosed in the UK annually (approximately 160 per day), accounting for 14% of all new cancer cases and ranking as the second most common cancer in men. Yet research is continually hampered by fragmentation of patient data. Although the NHS offers integrated care pathways, critical clinical information from urological and oncology notes to imaging and HRR gene biomarker results is often siloed across disparate internal systems. This fragmentation creates a critical barrier to evaluating care patterns and long-term outcomes across patient cohorts. Without harmonised data, key questions remain unanswered such as which patient groups receive biomarker testing, how PSMA-PET imaging (advanced medical imaging scans used to locate and track prostate cancer) maps to clinical outcomes, and how treatment progression patterns unfold in routine clinical practice.

Unlocking these insights requires fully integrated, high-quality real-world data across the entire NHS ecosystem.

Why this matters now: The rapidly evolving prostate cancer  landscape

The prostate cancer treatment landscape is going through unprecedented transformation. Over the past two decades, the field has seen the approval of life-prolonging agents with distinct mechanisms of action, novel hormonal agents, PARP inhibitors (targeted cancer drugs that block a specific enzyme called Poly (ADP-ribose) polymerase, radioligand therapies (which combine a cell-targeting molecule with a radioactive isotope), and advanced diagnostic techniques like PSMA-PET imaging. These breakthroughs have fundamentally changed how prostate cancer is diagnosed, staged, and treated.

This rapid evolution has created a critical evidence gap. The treatment landscape now includes options for disease stages that previously had limited available systemic therapies, such as non-metastatic castration-resistant prostate cancer (nmCRPC). Clinicians, researchers and drug manufacturers face pressing questions: What is the optimal timing for starting newer agents? How do different treatment sequences perform in routine practice? Which patients would benefit most from precision therapies like PARP inhibitors?

These questions cannot be answered by clinical trials alone. Randomised controlled trials take years to complete and often enrol highly selected patient populations. Therefore real-world evidence is essential to understanding how novel therapies are being adopted, sequenced, and are performing across diverse patient populations in routine NHS practice.
A new window into prostate cancer care

Flatiron Health’s UK Prostate Cancer Panoramic dataset changes that. Drawing from Flatiron's growing network of UK NHS Trust cancer centers, this dataset captures information from nearly 14,000 patients with prostate cancer, longitudinally from diagnosis through treatment and beyond, with the cohort expected to grow significantly over the coming year. Built on Flatiron's unified global prostate data model, it seamlessly connects into a global dataset encompassing nearly 400,000 prostate patients across the UK, US, and Germany to power cross-border research.

What makes this dataset powerful isn't just its size, it's its clinical depth.

It includes:

  • Full disease spectrum coverage: From non-metastatic presentation through to metastatic castration-resistant prostate cancer (mCRPC).

  • Comprehensive treatment history: Documenting exact lines of therapy (LOT), including hormone therapy, chemotherapy, novel androgen receptor pathway inhibitors (ARPIs), PARP inhibitors, and radioligand therapies.

  • Advanced biomarker and diagnostic data: HRR gene testing results, PSMA-PET imaging reports, and serial PSA measurements that reveal disease dynamics over time.

  • Longitudinal clinical outcomes: Real-world loco-regional recurrence, true clinical progression, drivers of treatment switching, and survival data.

This isn't just a snapshot of a patient’s journey, it includes their whole story. By enabling interoperability across data sources, Flatiron's Panoramic dataset allows clinicians and researchers to seamlessly analyse population outcomes and treatment patterns for patients with prostate cancer under their care.

What clinicians and researchers can now discover

With this integrated, longitudinal view, clinicians and researchers can unlock the scale needed to evaluate treatment patterns across diverse cohorts and draw on comprehensive, population-level trends to refine care models from initial diagnosis through to advanced disease.

For example:

  • Precision medicine: How do HRR biomarker status and specific gene mutations help us understand which patients could have been eligible for further HRR testing? Which patient subgroups benefit most from PARP inhibitors versus other novel agents?

  • Treatment dynamics: What are the real-world patterns of treatment sequencing? What specific clinical signals predict early treatment failure or prompt line-switching?

  • Disease biology and imaging: How do PSMA-PET imaging patterns and serial PSA trajectories correlate with progression risk and long-term response?

  • Comparative effectiveness: How do different treatment strategies perform outside of clinical trials, in routine NHS practice?

The UK Prostate Cancer Panoramic dataset is built on Flatiron's proven approach to real-world evidence: combining advanced machine learning data extraction techniques with deep clinical curation and continuous quality assurance. Governed by Flatiron’s VALID Framework, every variable undergoes multi-pillar verification to benchmark AI-extracted data points against expert human abstraction. This rigorous process ensures clinical accuracy and internal consistency, giving researchers and clinicians complete confidence in the data foundation as they evaluate population trends and refine care models.

What's next

The launch of the UK Prostate Cancer Panoramic dataset represents a major milestone in prostate cancer research, a resource built specifically for the questions that matter most in prostate cancer care, treatment optimisation, and patient outcomes.
